Prompt level is the most audited field in ABA

A note that records "60% independent with model prompts" survives review. A note that records "made progress" does not. Prompt level and percentage of independent responses are the two data points that let a reviewer verify that the service was delivered as authorized — and they are the two the form asks for first.

Supervision hours require supervision records

Supervision is only verifiable if the record shows what was observed and what directive was given. A note saying supervision occurred is weaker than one naming the competency observed and the specific task assigned before the next meeting. The supervision note tool generates both.

Risk screening belongs in the ABA record too

Self-injurious behavior, aggression, and elopement are risk items, not just target behaviors. When an incident occurs, the safety plan section documents what you did in response and who was notified — the part that is consistently missing from incident files.

Group and early intervention sessions need different structures

Early intervention sessions center caregiver training and natural environment teaching. Group sessions center the individual response within a group activity. Both have dedicated tools because a single generic form does neither well.
