Why group notes get denied
A note that describes what the group did is not a note about the person being billed. Reviewers need an individual response statement — what this specific client did when the group intervention was delivered. Write the group activity once, then the individual response, and generate one note per member.
The efficiency trick most clinicians miss

Group size and billing
