What supervisors forget to document
Most supervision notes record that supervision happened. Fewer record what competency was observed and what directive was given. When a certification board or a payer reviews the file, the second category is what demonstrates that oversight was substantive rather than nominal.
Observation of the supervisee, not the client
A supervision note is about the supervisee’s clinical behavior. Describe what you observed them do — data collection accuracy, prompt delivery, parent coaching — and what you said in response. Client progress belongs in the client’s record.
Give the supervisee something specific
The plan section generates a directive statement. A supervisee who receives "continue to work on programming" learns nothing. "Collect interobserver agreement data on two targets before next supervision and bring the graphs" is a directive that can be verified at the next meeting, which is also what makes the next supervision note easy to write.
