How this works
Screen every session, document most sessions
Not every session needs a full safety plan. Every session needs evidence that you screened. A short statement that screening occurred and no indicators were present is complete documentation — and its absence is what creates exposure.
When indicators are present, the response is the record
The note has to show what you did after identifying risk: who you notified, what was recommended, when follow-up is scheduled. The plan section generates those lines from your selections so the escalation path is visible in the note rather than reconstructed from memory later.
Keep observation and judgment separate
State what the client said or did, verbatim where possible. Put your interpretation of risk level in the clinical reasoning section. Mixing the two is what makes a risk note hard to defend, because a reviewer cannot tell what was observed and what was inferred.
What this tool does not do
It does not assess risk for you and it does not decide level of care. It organizes what you already determined into documentation that holds up. If you are making a level-of-care decision, follow your agency protocol and your licensure requirements.

Common questions
Is this a suicide risk assessment tool?
No. It is a documentation tool. Any validated risk assessment instrument should be administered and scored separately, and its results entered in the observation field so they appear in the record.
Who should see a safety plan?
Follow your agency policy on distribution. The generated note is written for the audience you select — supervisor, referring physician, or caregiver — and the language changes accordingly.
