Frequency is the part people get wrong
Home programs fail when the prescription is unrealistic. Five times a week for four weeks sounds reasonable in a clinic and collapses in a living room. Set the frequency to something the household will actually do, and note in the program that shorter and more often beats long and once.
Give them a way to know it is working
Caregivers stick with a program when they can see change. The goal section states the criterion in plain terms — how many times out of ten, with how much help — so the person at home can tell whether today went better than last week.
Write it for the person who will do it
Set the audience to Parent or Client and the language drops clinical shorthand. If the program is going into a chart, generate a clinical version as well — the two documents serve different readers and should not be the same file.
Pair it with the session note
Documenting that a home program was provided and reviewed is part of the skilled service record for most payers. Generate the note first, then the program, and both documents reference the same interventions.
